Time for consult: I would have him seen by his PCP and evaluated for chronic constipation/stool retention. If your PCP is unfamiliar with the issue, contact a pediatric Gastroenterologist or developmental pediatrician.The various problems that lead up to this symptom are complex, and often take months and years to resolve with proper help.

Prevent constipation: The common cause for this is chronic constipation. The child withholds the hard stool due to fear of pain , which in turn makes the problem worse. A balanced diet ,with an adequate intake of water , fiber ,and avoidance of excess milk intake , would help. Periodic use of stool softeners may be needed . Sometimes emotional stress could trigger this , especially in school age children
