Your : Your child probably has fecal retention syndrome. He is essesntially holding his poop. The best treament for this is a laxative called miralax (polyethylene glycol). He should take about 1/2 to 1 capful per day for up to six to nine months. It is important for him and his poop to part as friends for a long time. The reason his stool is in his underwear is not that he is not wiping well is that he is having breakthrough or fecal soiling from the liquid poop going around the hard, large stool in his colon.If the symptoms continue you should see your local doctor.

I : I agree with dr. Misch. If his issue is limited to soiling his underwear that is one problem. If he is actually defecating in his underpants and bypasses the toilet altogether that is a different matter. If it is the later and he checks out medically by his doctor, there could be emotional issues impacting him (especially if he has had good bowel control in the past). Children with adhd can sometimes get so focused on an activity (like video games or cartoons) that they fail to pay attention to important things, like needing to get to the toilet really soon. By age nine, it is probably some what distressing to him to have poop in his pants. I know it has to be frustrating as a mother, but try to be patient as this is being worked out. Good luck.
