Question ?: If the first molar is abscessed, then it needs either root canal therapy or extraction.. no other choice. It is best to try to save the tooth. Appears root canal therapy is indicated.

Abcessed first molar: The worst thing you can do is to extract the first molar. This is the anchor tooth for chewing and straight teeth. Have the root canal and save the tooth, and prevent future problems.

If possible...: The best alternative would be to save the tooth with RCT and a restoration. The second molar rarely if ever exactly replaces the position of the first molar. If the 1st molar was extracted, the space would have to be retained until at least 18 years of age to consider an implant. If the RCT fails, you could always do the implant restoration later on. I would recommend an Endodontist do the RCT.

Root canal: You must see a specialist - the Endodontist- for the root canal if the tooth is restorable. Not advisable to extract, as he is too young for an implant and the space would create major problems in the future. If the tooth can last another 7-10 years, then an implant would be best if needed. Important to save the bone, especially as it is growing. See Prosthodontist as well for crown. Good Luck.

See your DDS: Sounds like you are getting good advice, The pending eruption of the second molar has no bearing on the proposed treatment plan. However if you do not choose to treat the existing infection that could adversely affect the new tooth trying to erupt into position. See your DDS to get the best care for your child.
