Bacteria can enter : From the external auditory canal ;#40; as with q-tip use;#41; or from inside out from the nasopharynx. Pressure equalization tubes keep barometric pressure in the middle ear the same as ambient barometric pressure. If there is an overt or submucous cleft palate or if the baby drinks formula or juice in supine position, reflux of liquid into the middle ear can occur despite pet's. See my comment for more.
