A 21-year-old member asked:

does cerebral palsy get worse over time?

Generally not: By definition the injury causing CP creates a stable effect; that is you do not see deterioration of skills after they are acquired. Some nervous system disorders get worse with age as the injury is continuing, not cp. Some CP patients may choose a wheel chair later in life although they can still walk some for the convenience & speed. Most CP kids actually will get better over time with therapy.

It should not: Cerebral palsy by definition is a static encephalopathy. This means that it is a stable condition that just needs help to overcome. If there is progressive loss of function, then some other diagnosis must be considered. In some patients the back, hips, ankles get tighter and contracted. This is not worse disease but just progression of the spasticity.

CP acquired< 3yo: By definition cerebral palsy is related to injury to the developing nervous system befort the 3rd birthday. (pregnancy, labor, delivery or the infancy years). Brain injury after that point is looked on as a different process similar to adult strokes or brain injury defects. The separation is more than random. Infants with CP have a greater recovery potential than adults with similar injuries.

Can be improved: The wonderful thing about working with kids is their ability rewire past some of the injuries of cp. Pt/ot/speach therapy and other modalities can minimize the impact. He injured areas if major will always remain injured, but plasticity is a word applied to their response. Adults with the same injury don't as much ability to regain functions.

Multiple: The risk of CP is higher in several groups: premature (<32 wks);small for dates (<2kg); brain hemorrhage;multiple birth;low oxygen levels at birth;shaken baby syndrome; meningitis; intrauterine infection; near-drowning; carbon monoxide poisoning. All settings where the developing brain can be injured.
A 21-year-old member asked:

What is patterning, and how does it relate to cerebral palsy?

Dr. William Goldie
Pediatric Neurology 48 years experience
No relation found: Patterning was a popular therapy method about 40 years ago. The insitute of human potential used this method to "cure" cp. It was never found of definite value when examined critically. Appropriate therapy methods are very important, but intense patterning is not worth the enormous time and cost.
