1/23/14 diag w non occlusive dvt. put on xarelto had repeat ultrasound. no change in clot. why? medical options? vascular surgeon? more xarelto?

Chronic DVT: Sometimes, this could have been becoming chronic laminated thrombosis, and can cause something known as post thrombotic syndrome (leg swelling and pain). If us did not show any new clot, it did not mean xarelto failure. I recommend you see vascular surgeon. Usually post thrombotic syndrome can be effectively managed by vascular surgeon (intermittent wrapping). See your doc. Hope helps.

Pharmacomech lysis: Xarelto doesn't dissolve dvt. Its goal is prevention of clot extension. It requires your body to heal the clot. Thrombolytic drugs e.g. Tpa (alteplase) dissolve clot. Xarelto is not a thrombolytic drug. Pharmacomechanical methods can both dissolve clot and remove the obstruction of blood flow that they cause. These methods are performed by endovascular specialists, typically interventional radiologists.
