Advise dialysis,,: At this point, dialysis is likely a crucial thing to do to help filter the blood and lower the BUN and creatinine. It would be best to see your doctor and review the possible methods that would be best for your father and get treatment soon. Best of luck.

For some time yes!: Ask your doctor the eGFR or CrCl (creatinine Clearance) of your father,i.e. the rough estimation of his kidney function. The elective initiation of dialysis is planned when that number drops below 15cc/min, to improve survival and general sense of well being. Otherwise in the absence of complications(high potassium, etc), controlling diabetes and hypertension can delay but not prevent the dialysis

Depends . . .: I agree w/all my colleagues' answers but I'm chiming in b/c it's important that your father tell you (plus rest of family) what he wants. If he's got a number of other medical problems and is in poor health, he may not want to get dialyzed 3x/wk. If he's otherwise healthy & has some long term goals in mind then he may want to consider planning for dialysis. In meantime, control BP & sugar.
