A member asked:

My father in law has alb/ creatinine urine test result of 4,291. he is being treated with lasix.his gfr is normal. blood results show bun is 28. bun/ creatinine ratio 27. can he still have significant kidney failure with normal gfr?

He's spilling large amounts of albumin in his urine indicative of glomerular damage, ? from hypertension, diabetes, light chain nephropathy or glomerulonephritis. His renal tubular function is apparently still healthy enough to maintain a normal GFR and serum creatinine. He should be under the care of a nephrologist.
