Normal CD4 count: That's a normal count, so middle ear infection micro-organisms would be expected to be the usual ones: strep pneumonia, hemophilus influenza, moraxella catarrhalis, strep pyogenes. It is doubtful such a patient would need any different antibiotic than one without hiv.
