Usually on the Left: Congenital diaphragmatic hernias occur on the left 85% of the time and are posterior (bochdalek hernia), most the rest are located on the right posteriorly. About 2% occur anteriorly in the middle (morgagni hernia). Adults can get hiatal hernias on the left, because that is where the esophagus passes through the diaphragm to reach the stomach. Trauma and surgery can cause diaphragmatic hernias too.
