Significant: At your age if you have never had a colonoscopy you need one now! the blood could be secondary to the nsaid's but then your stool would most likely be black. Hemorrhoids are a possibility but you must rule out cancer as you are in the right age group and i don't like the symptoms of back pain and bleeding.

How long?: Left sided pain and blood in the stool suggest a left colon pathology. The differential includes diverticular disease, tumor, or colitis. Other causes of blood in the stool unrelated to back pain are hemorrhoids. I would recommend seeing a primary care or GI doc this week if possible. If pain worsens, you start running fever, start having nausea, or the bleeding gets heavy you go to er.

See doctor: Will need a colonoscopy because of the rectal bleeding.Have to worry about colon cancer as a cause. Could be colitis, polyps, hemorrhoid, or from the nsaid. In the meantime stop the NSAID till you see your doctor.
