Depends on symptoms: It depends on what your symptoms are and how much it bothers you. If you have no pain, or limitation of opening - no treatment indicated. If you are having troublesome symptoms, i would start with conservative treatment including otc pain meds, soft diet, jaw exercises. Splints often help. If you've tried conservative treatment, you may benefit from steroid injection, arthrocentesis or athroscopy.

TMJ Disc perf: For a 27 year old to have such a serious tmjoint condition warrants an evaluation with a team of knowledgeable and experienced doctors in this field. If i were in your shoes, i would research the best omfs, orthodontist, prosthodontist and tmd specialist in your area. A local teaching hospital might be a good place to start. Be informed, learn all you can and get a consensus of opinion.

Not a whole lot: A perforated TMJ disc cannot be repaired and surgery to replace the disc has mixed results. If you are unable to function with pain or difficulty then surgery may be your only option.
