This is serious.: After all this time on appropriate medical treatment you need to have a discussion with your doctor. The time may be ready for surgery 10 liters a week is a lot to lose. Get to your doctor asap for a discussion of the possible need for surgery.

Tertiary center: I presume that your surgeon and family doctor have tried without success in getting this stopped. I suggest that you consult an university center near your home to find out what else can be done.
