See details: Dizziness is not vertigo. If the room spins, it is vertigo. Each has different causes and different treatment option. Have your brother see an ENT specialist if he has true vertigo.

If all other workup: Is negative, and its not heart either, there are some unusual things that can cause dizziness, including cervical paraspinal spasm and ear accumulation. Ask doc to check his ear canals (if they haven't) and also see if he has any hypertonic musculature in his neck. If he does, consider manipulation or massage and see if it helps. Have they done the dix hall pyke maneuver on him?
