Prob. liver disease: It is impossible for anyone here to say without examining him but given this limited information i would guess he has ascites which is fluid retention caused by cirrhosis or other serious liver disease which may well be related to his history of heavy alcohol use. This is a serious problem. He should see a doctor as soon as possible. Ayurveda and homeopathy may also help some.
