Can i get an easy to understand explanation of septoplasty?

Reshape the septum: "plasty" means to change, as in the shape. Septoplasty is a surgery where the nasal septum is reshaped to improve the nasal airway anatomy.

Why would a septoplasty require external incisions? My surgeon says my septoplasty will be complicated, and he might need to use an external incision. Everything i've read says incisions are always internal. I didn't understand how he explained my case. W
