RCT lesion: I would consult with the dentist who performed the root canal procedure or an endodontist about the possibility of need for retreat. Are there any symptoms or just the spot on the radiograph? Was it there at completion of the initial root canal treatment?

Compare x-rays: I would want the dentist/endodontist to compare x-rays then and now to provide a more definitive answer. Sometimes the lesion can be healing granulating tissue. A lot depends on if you have symptoms too- swelling, bump on tissue, pain upon eating, temp sensitivity.
