Is a biopsy routinely done during an endoscapy to rule out problems secondary to long standing chronic reflux when there are no visible abnormalities?

No: During colonoscopy, a biopsy is performed only if there is a visible abnormality. Biopsies can lead to perforation and bleeding and should be performed only for a good indication.

Yes: It very common to take random biopsies of that part of the esophagus to look for microscopic changes that might not look abnormal to the endoscopist.
