Fear: Talk to the phlebotimist and ask her to apply a local anesthetic to the site where blood is drawn.

Ask your doc: For one tablet of xanax (alprazolam) .5 mg to 1.0 mg depending on your size to be taken 45 min before the blood draw. Have someone drive you there and back. Best wishes.Long term think of seeing a psychologist to overcome the phobia.
