Diagnosis ?: What did your dentist say to you? Your dentist has had the opportunity to examine you, test the tooth, take a radiograph, etc... Over the internet, no one else has that luxury. Can't possibly guess at your problem.. That won't help you. It might be wise to consult with a local endodontist.

And ????????: What did the dentist say as possible cause? If unhappy, unsatisfied or just not sure, consider seeing another dentist for a second opinion or an endodontist to have that tooth evaluated. Sometimes we are unsure of the cause of tooth pain when clinical exam and x-ray is not definitive. We then have to wait or jump in and do rct. If right, patient is happy. If wrong, patient ; doctor are not.
