Could be: Pulpal infection can result in a bone loss or a cracked tooth. Consider an evaluation and xray exam to confirm that root canal therapy is successful and that there are no other complications

Many possibilities: It could be any number of causes. Fractured root, occlusal trauma, periodontal gum inflammation, periapical infection, failing Root Canal Therapy, unfilled accessory canal? All possibilities. Requires clinical and radiographic evaluation. Consider Endodontic consult if necessary.
