Pictures / $$: Try showing him some pictures of the results of negligence -- rotten teeth, missing teeth , bleeding gums, collapsed face, etc. Also mention horrible breath, unattractive smile and gums, and systemic damage to other organ systems. Also mention how much it can cost to repair and replace damaged and missing teeth and lost bone. If he loses too much bone, he may become a dental cripple in later years

Does he care ?: Meaning, does he care about his appearance? Consequences of neglect? Is he in a relationship? Self esteem? Employment? You can try to reason, but I wish you the best. I went to a patient's wake today; she died from cancer, and continued to smoke after being diagnosed and all during her chemo and radiation. Go figure.
