Yes: Breast cancer does not act in a predictable fashion. It can metastasize to distant body sites without involving lymph nodes.

Yes, but unlikely: If your lymph nodes were truly negative, it's much less likely you would develop metastatic disease, but it happens. Breast cancer that is er positive may show up many years later in the bones.
