I got this report from my radiologist , what's the problem? do i need surgery?

Was report required?: In getting a radiologist report one needs to assess 'what was the clinical concern?' and does the report provide an answer? If not, why not? Technical reasons? And if no answer, prproposed tests that could answer the question. Without knowledge further of the concerns, i cannot determine if further if more workup is neede.
