Heart disease: You are not specific as what kind of heart disease? If it is heart failure you may want to know the new york heart failure classification which would give you an idea about the severity of disease and prognosis. Please have cardiologist on board.

When you were: In hospital you most probably had complete work up to determine what the diagnosis was? Get appointment with cardiologist that saw you! or contact your own pcp.
