Several: You may have an interstitial nephritis from illness or medication preventing you from concentrating urine. Your Accutane may make you feel dry. If you're well-muscled, this may explain high creatinine -- when i was lifting daily, i ran 1.7 mg/dl. If you do take Ibuprofen or similar meds, drink plenty of water. A majority of these consults result in reassurance but let's be certain.

Elevated creatinine: Creatinine comes from muscle. Some very muscular men will have some elevated creatinine with normal kidney functions. Otherwise, you need an evaluation with urine testing for protein and red blood cells, a kidney ultrasound, and blood pressure check.
