Is it possible to get a communicating hydrocelle as an adult, doc says there is no sign of one, i strain a lot on the toilet, tested swell and go down?

Get an exam: If you see something swell and go down again when you bear down, you need a thorough testicular exam and hernia check. Those are more likely than a communicating hydrocele at your age. There are other more benign possibilities, but those are the most likely.

Varicose Veins: Sounds more like enlarged veins of the spermatic cord. Similar to varicose veins of the legs or the rectum. If very large and persistent can lead to a decrease in viable sperm level and difficulty in having children.
