See details: It is difficult to give specific advice without being able to visualize the shoulder. The one thing i can say is this does not sound like a strain. You need an orthopedic evaluation if you have not had one already. Given the severity of the problem a more thorough evaluation is essential and an MRI should be seriously considered.
