Can anxiety creates heart attack in a 20 year old male?

No: Kitten, anxiety can imitate anything but it usualy does not hurt. It does not mean it should not be treated.

Unlikely: Stress can cause the heart to work harder but to cause a heart attack directly..Unlikely. When the heart works harder, and if one has a clogged coronary, then may be a heart attic ensues. But this is unlikely in a 20 year old unless the same 20 year old is using hard drugs such as meth or heroin etc. If not, i would not "worry".
