Get evaluated again: You may have a jugular tumor -- this would be a common presentation. Needs to be ruled out in any case. Good luck.

Ear thumping: Hello, it sounds like your pulsatile tinnitis has returned. Depending on what type of ear surgery you had, pulsatile tinnitus may be treated by simply replacing extruded tubes from your ears if that's what was performed in the past to relieve your problem. The cause was probably fluid accumulation in the middle ear space.
