Carefully!: First with local anesthesia. Then with special instrumentation the nail can be removed. Sometimes borders only are removed and sometimes a whole nail can be removed.

See a podiatrist: Not all medical doctors have the right instruments to perform such a procedure. Have it evaluated by your podiatrist as they are well equipped and experienced for such procedures. First, the toe is anesthetized. The nail is then removed using specialized instruments.
