Depends: Depends on what the cause of the distended stomach is. If it is from excess gas, you need to find out if it is a milk intolerance, a simple sugar intolerance, and overgrowth of bacteria. There are many causes. I would suggest starting with a evaluation by  your physician. Some stomach intestine specialists, can do breath tests to determine specific causes of distended bellies.

Probiotic: Probiotics are usually dosed once a day, with a meal.  Make sure you have a brand that delivers active colonies to you.  If you have gastritis, the probiotic may not help that by itself.  I see you're on omeprazole. Hopefully your doctor has tested you for h pylori which is 1 cause of gastritis. Ulcers can also cause pain with meals, and shortly afterwards as well.

No: Injection would not increase your girth size overall. A good way to reduce the girth size would be cutting down on carb , alcohol, and fat intake while exercising regularly (core exercise especially).  Cigarette smoking would shift the fat to the belly.  As you know already, not eating more than you need is essential both for your diabetes and for your girth. Good luck.

Flat belly: Dont fool yourself. Eating whole wheat bread is no guarantee that you will have a flat belly. Daily exercise along with proper diet would be more helpful.

Both: When choosing carbohydrates like bread, rice, rolls, and even cookies, choosing whole grains (including whole wheat) will help improve your health and reduce the development if a big belly.  The less processed a food is generally the healthier. So a baked potato is healthier than mashed or fries. Brown rice is healthier than white. Whole grain bread is better than white.
