Discuss with his doc: He may not need tests but your physician will want to ask a few more questions to decide if there is a concern. Rr 40 is fast  even with a fever if he is at rest and fever for 13 days is concerning. Good chest xray does not prove anything.

Not common: Cancer from smoking is less likely after 10 years of smoking, however, recent studies have shown that chest ct can discover cancer earlier than chest x-ray. Ask your doctor if you are a candidate for the screening. The scientific pool of patients were older than 40 years old.

No one test: Pneumonia diagnosis is based on CXR findings, identification of a causative organism , and by ruling out other causes of symptoms, e.g., heart failure. It is not a do it yourself issue.

i am a medical assistant and had my tb test come back positive but cxr negative. is there any follow up i should do?

Yes: For a person with a positive PPD skin test and a negative chest x-ray, if he had a BCG vaccine in the past, he can do a Quantiferon TB blood test to see if he was exposed to real TB or just to the BCG shot. If he never had a BCG, he can talk with his doctor to decide if he should take INH pills or do the TB blood test. If it is not certain he truly was positive on the skin test, he can repeat it.

Probable UTI: The symptoms to me sound like a kidney infection. Make sure that that is evaluated and has not been ignored. Otherwise i will leave other comments up to my internal medicine colleagues who i'm sure will have other input.

ALTE: Alte (apparent life threatening event) is an acute, unexpected change in an infant's breathing behavior associated with 
apnea( no respiratory effort ), 
color change ( cyanotic or pallid), 
marked change in muscle tone & 
choking. Only half of the cases, cause (s) is found, e.g., gastroesophageal reflux, seizure.  There is no causal relationship between alte and sudden infant death syndrome.
