Upper limit: If you have a very low level before treatment your doctor will prescribe a high dose for a month or two. When you levels are back to normal you should do well with 1000 or 2000 units a day. Unless advised otherwise by your doctor don't "overdose".

Get blood level: The good news is that you don't have to guess. You can measure your Vitamin D level and then go from there.  While most adults can safely tolerate 1000-2000 IU/day or more, toxicity is possible in the long run. So, it is best to find out what your starting point is.  Also, if you are in the sun a lot you may not need as much in supplement form. However, sun alone may not be enough in many people.

Time To Get Healthy: An excellent  and relevant question. And I suggest you do some extra research online. But as far as I know, it's more what you should take as opposed to not. Because of certain infection and immunity precautions I'd suggest a healthy diet (read: Spartan, read veggies, lean protein, etc) and a multivitamin and CoQ-10 wouldn't hurt. And most of all, a positive energetic attitude (an immune booster)

??: There is certainly some controversy concerning this issue but i will say that I have personally taken ~100 mg of vit b6 daily for about the last 30 years or so and I do not believe that it has caused any sig harm and it seems to help with my carpal tunnel symptoms.

Assume that: this is a weekly dose - not a daily dose. That is another way to prescribe it.

Ca and Vit D: Unless you have money to throw away or you have problems with Ca and or Vit D I would not take these. Always discuss with your team.

Folic Acid: Has been shown to be beneficial for prevention of birth defects in dosages up to 4mg/ day.  Beyond that isn't studies.  All things can be toxic at extremely high dosages, however you are safe if you are in this range.
