This is tissue pain: Without knowing the site or quality of the pain, consider mild inflammation of the soft tissues or of the joint capsule.  If you sleep with your arms over our head, you could be putting traction on the nerves to your arm; if your neck is over extended or fully flexed the cause of  the pain could be in your neck warning to see  your doctor so that the problem doesn't get worse..

so i woke in the middle of the night with severe right arm pain with nausea it lasted about 25 minutes later on about 6 hours later then pain came back for about 2 hours now again currently it back could i possibly be having a heart attack?

: Please see your physician. These symptoms may indicate a cardiac condition, and the cause of the pain should be determined. Watch for dizziness,shortness of breath and avoid meals two hours or less primary prior to bedtime.

See details: It is difficult to give specific advice without being able to visualize the shoulder. The one thing i can say is this does not sound like a strain. You need an orthopedic evaluation if you have not had one already. Given the severity of the problem a more thorough evaluation is essential and an MRI should be seriously considered.

Many things: He could have a frozen shoulder, a rotator cuff tear, bursitis or even a neck issue.

Have him see a shoulder specialist, often a good physical exam can give him a diagnosis and then a treatment plan.

Possibly: Severe coughing could conceivably lead to a cervical disc herniation which could then cause bilateral arm pain.  This can be diagnosed on mri, so see your physician if symptoms persist.

Not usually: That sounds like a musculoskeletal problem, not a viral infection.  The exception would be shingles, which could give you shoulder and arm pain in the setting of a rash with blisters.

No: It depends on the size and aggressiveness of the tumor, and how badly the nerves are irritated. Rapid severe pressure or inflammation can cause more arm symptoms. On the other hand, slower growth and gradual nerve pressure may cause little to no problem.
