Many things: He could have a frozen shoulder, a rotator cuff tear, bursitis or even a neck issue. Have him see a shoulder specialist, often a good physical exam can give him a diagnosis and then a treatment plan.

Rotator cuff: Sometimes rotator cuff problems will cause pain at night since the gravity has been eliminated. Sitting up allows gravity to move the cuff away from the overlying bone(acromion).
