Cast: Sometimes just resting the foot in a cast or walking boot can help in a unknown healing process.  I would seek a second opinion and evaluation for further care and treatment options.

Stop Playing around.: Sounds like there's a whole lot going on here. Any one of the areas of pain could be the problem and compensation causing the rest. 
Could be soft tissue or bone or both. Your best bet is to have it examined to eliminate the possibilities. My guess is that you will need a cam walker of stabilize the foot for a while.

Have you tried thera: Could be tendonitis which may need anti-inflammatory meds and physical therapy.  Have an MRI to determine what cause is.

terrible foot pain on top of foot went to see an orthopedic. doctor bought some inserts but they are not helping sometimes i have to drag my foot feels like the bones in my foot are coming apart please help?

May be a nerve: I would be more concerned about dragging the foot than pain. If you are having weakness then you may have a nerve problem in the leg or possibly even from the low back. It sounds like you need a better work up. I would discuss a consult to a physiatrist or neurologist. If the work up leads towards an intrinsic foot problem then you may be better served by a podiatrist.

top of foot pain in bone, center of top of the foot.  no impact/trauma. the pain is very mild and i can exercise thought it.  i am very active-run, play basketball on hard courts (concrete), etc.  im 46.  is this possibly some stress fracture?

Pain: if you can still walk on this it's unlikely to be a stress fracture. Common reasons for foot pain would be an Extensor Tendonitis perhaps some degree of neuritis of the nerve of the top of your foot.

If your foot or ankle is still hurting after 1 month, it is likely fractured and missed on the initial XRAY read or re-injured due to improper immobilization of the original sprain. Your foot needs re-evaluation, a repeat XRAY, and a proper orthopedic shoe with Ortho follow-up +/- Physical Therapy. See your doctor today.

Hyperuricemia-gout: Gout typically effects the big toe joint (mpj) due to sodium urate crystal deposits and due to gravity of the joint. It will present as a red hot swollen joint that you can barely touch. If you have pain on the side of the foot it probably is more biomechanical in nature.
