Tough question: Have you had several ovarian cysts during your lifetime? Are there any problems with the uterus such as fibroids, or prolapse? Have you had multiple surgeries in the past? These are all factors to help you make a good informed decision with your gynecologist about what to do, so go talk it over and listen to their advice.

Previous scarring: Previous surgical scar tissue (adhesions) make this a more complicated question to answer because the subject ovary may be "stuck" to other tissues like bowel, pelvic sidewall or bladder. If you don't want to return to surgery after this operation you might give consideration to hysterectomy with ovarian removal.  You will need a very competent surgeon to undertake this operation- interview wisely

Pelvic pain=get seen: First I am sorry you are hurting.
Lower abdomen pain can be due to several sources including kidney stones, appendicitis (is your pelvic pain on the right?) or ovary cyst rupture.  All are very painful.   Usually with a confirmed ruptured cyst, time will heal and you can take pain meds (otc tylenol/aleve) or get a prescription.  A long term solution could be surgery (i don't recommend @ this time).

Cyst: Cyst on ovary may be a type that goes away on its own - a functional cyst - or a benign growth that will need surgery to remove. Seeing what the cyst does over the next few months will show which it is. The blood tests show you are getting near to menopause but not there yet.

The same: The uterus is not an endocrine (glandular/hormonal) organ and so removing it does not change the endocrine environment in a woman's body. Not all ovarian or breast cysts result form hormone exposure, but many do.

Cyst: You should know the reason before doing surgery.
Small 2-3 cm cysts appears in functioning ovaries every month and disappears with ovulation. These small cysts are not painful.
But there are larger and different cysts, like hemorrhagic, endometriosis that might be painful and needs to see your gyn for its management.

Adhesions: It sounds like you may have developed adhesions. Adhesions are similar to scar tissue within the body, which can make future surgery around the area very difficult. You might consider seeing your doctor to determine if there is any other medical way to treat your ovarian cysts.
