Subtle difference: Citalopram stops serotonin from being recycled in the synapse, while Mirtazapine stops the feedback loop causing more serotonin and norepinephrine to be released into the synapse.

They are different : Mirtazapine is a dual acting antidepressant that blocks the reuptake of both serotonin &amp; norepinephrine (SNRI) while citalopram is a selective serotonin reuptake inhibitor (SSRI). Discuss with doc before making any chamges

Celexa/Remeron: You might, if it is ok with your doctor, try 15mg. Remeron (mirtazapine) at bedtime instead of the 30mg. The lower dose is better for sleep.

Either one or both: Both medications can treat panic attacks and anxiety.  They can also be used together (usually citalopram during the day and Mirtazapine at bedtime) and work synergistically to decrease panic attacks and anxiety.

Talk w ur doc first: Both citalopram and Mirtazapine are both antidepressants. Mirtazapine has a side effect of making a person sleepy so it sometimes is helpful in this regard. However your doctor may not want you to take two antidepressants so you should discuss this with him or her before you make that decision.

Add CBT therapy: For decades research has shown that optimal therapy outcomes require therapy - it enhances medication and for some is sufficient alone. The need to learn symptom mgmt skills cannot be met w/out it. Don't lose hope - it is also a major part of successful treatment. In fact, w/out confidence it is unusual for any treatment to be a success. So, find a therapist you trust and work at it! Best!

There is no 'should': Certainly not day by day will you see changes. If you work with the doctors and a therapist and the medications are right for you, you will improve. But it may have ups and downs and may plateau at times. It's not only the pill - it's what you do too. Talk to you doctor about how to manage your expectations - don't let yourself be disappointed.
