Big toe: You didn't state what treatment(s) you have had.  The edb  isn't really involved here  - see your podiatrist or physician who has been treating you as pain and swelling this long after is unusual .

Conservative tx : Should always be tried first. At w hat point does one decide it is not working sounds like te question....I think as long as you are progressing you stay the conservative course. When the treatment is not going anywhere, you may consider changing the tx. But, i would change to a diff. Form of conservative tx before rushing into surgery. Exhaust your possibilities before looking for surgery.

Maybe: Mri would help to determine how much the tendon is seperated. If the ends are close together, then casting for about 6-8 weeks may be all you need. Surgery might make the tendon heal faster probably but not always. Have a podiatrist or orthopaedic doctor evaluate you for the best treatment option.

Talk to hand surgeon: There is no medical treatment for a tear unless it heals spontaneously.  Obviously avoiding further stress on the area with a splint to allow the tendon to heal (only it is a partial tear). The best approach would be to consult a hand surgeon before further damage occurs.

Usually not: But you need to rest that, and may need splinting or bracing. See your doctor to assure the proper treatment plan.

i have been in a tempory cast for 1 week for a extensor digitorum injury it is no better will they put me in a regular cast?

Too early to tell: If it is properly fitted for you/your case, the temporary cast should work well. Casting basically works to immobilize the area and protect it from further injury. Healing is from your body and not the cast. Tendon injury may take many wks to heal and so 1 wk is too early to tell. Close follow-up with the doc is recommended for reassessment/care. Good luck.

Why not wait: Nd while waiting get a prp injection and keep your foot in a cam walker. What conservative treatment have you tried to date?

Bracing: You may need to rest and brace your peroneal brevis tendon and let it heal first.  Although exercise and massage may help strengthen the muscle, your tendon may have been damaged.  Lastly, if the damage is an extensive tear, you may need repair of the tendon surgically.  I hope that helps.  Good luck!

Neuritis: Nerve inflammation from either compression or overuse. You can try OTC anti inflammatory medication such as Advil or Aleve (naproxen) for the next week; however, if not improved, see your physician.

what are conservative treatments for a torn peroneal brevis tendon? i've done pt and had an injection both unsuccessful. insur. comp. denies surgery.

Cause of tear....: This is difficult to answer without knowing the time frame of when the injury occurred and when the various types of  treatments were tried as well as the severity and location of the tear. Ill timed p.t. and steroid use can not only  be unsuccessful, but, in some specific circumstances, can actually delay the healing. It might be time for another opinion.
