At what point should surgery be performed on planter facitis and 2.3 brevis split tear.

Conservative tx : Should always be tried first. At w hat point does one decide it is not working sounds like te question....I think as long as you are progressing you stay the conservative course. When the treatment is not going anywhere, you may consider changing the tx. But, i would change to a diff. Form of conservative tx before rushing into surgery. Exhaust your possibilities before looking for surgery.
