If it is a chronic issue, you should initially try increasing the amount of fiber that you take in, and adding a fiber supplement such as Metamucil. Immediate exam: You will need a rectal exam to check for inflamed or infected hemorrhoids, fissures , cuts, fistulas, colitis or polyps. You may need anoscopy. sigmoidoscopy or colonoscopy. You can try using Anusol HC ( hydrocortisone)  rectal cream or suppositories. You must be seen by a gastroenterologist. DO NOT IGNORE THESE SYMPTOMS!

Difficult BM's: Drink adequate amounts of fluids daily and eat good amounts of fiber found in bran cereals, vegetables,salads and fruits and/or take metamucil daily. For very sluggish bowels, a daily stool softener like colace helps but some people need a gentle stimulant like senokot (senna). If you have the urge and can't move, insert a glycerin rectal suppository or, if needed, a dulcolax rectal suppository to empty..

Constipation: First, digitally removing stool is not a good idea. Trauma to the anal opening and rectum notwithstanding, pain and worsening discomfort may make the problems continue. Second, not every patient has a bowel movement 'every day'. Your regularity is unique to you (and your diet). Finally, if this problem has persisted for longer than a few days, see your doctor to discuss.

Work from below: When you have a strong urge to have a bowel movement and can't, work from below. Try inserting a glycerin rectal suppository or a dulcolax suppository. If this is unsuccessful, administer a fleets enema. You could also try a low tap water enema. If these fail you may have to be manually disimpacted by a professional especially if you have abdominal discomfort or distension.

Ans: Well he may have 2 issues here. The malaria is a problem of its own. There is medication but still will get severe flare ups. The urine issue he needs to see the urologist first. The incontinence is an issue get evaluated

Polyethylene glycol: This agent can take up to 1 to 4 days to have bowel movement.   If have stool impaction, you might want to give yourself an enema first, then take polyethylene glycol, one tablespoon in 8 ounces of water up to three times per day.   Should eat high fiber diet with adequate water.   Citrucel, mix 8 ounces of water daily.
