Even : Even at the high doses of Aspirin that you are taking, there is no substantiated medical evidence that this is useful in preventing pulmonary embolism or deep vein thrombosis.

Aspirin is a highly effective anti-platelet agent when taken in low doses (e.g. 81mg a day) and this means that it is effective in preventing arterial blood clots, such as those that cause a heart attack or stroke. It is not a reliable agent in trying to prevent venous blood clots such as deep vein thrombosis (DVT) that can embolize to the lungs (pulmonary embolism).

Taking such high doses of Aspirin every day for headaches is highly concerning and may be causing you more harm than good. I recommend talking to your doctor about your headaches and medication use.

D-dimer: Yes a d-dimer can miss a PE if the clot is small or if the test is done too soon or if the test is done incorrectly. The diagnosis is a combination of history, exam, labs, and radiology studies as well as clinical judgement.

Different: Pulmonary embolism can cause chest pain, shortness of breath, cough or even fainting. It is a complication of DVT which has different symptoms. Often, there are no symptoms of  dvt. Usually leg swelling, pain, redness and warmth accompany a DVT when it is symptomatic. Sometimes cramping or a feeling like a muscle strain may be present.
