i have a complex cyst in my ovary measuring 6.5x8.9x4.5 cm with two nodules with the bigger one measuring 1 cm. should it be removed and how?

Recommend excision: Complex cysts should be surgically removed in the large majority of cases as the concern is whether the cyst is benign or malignant. The large majority are benign but there aren't any good tests to make that determination without actually removing the cyst. You should consult a gynecologist and he or she may refer you to a GYN oncologist to do the surgery. A laparoscopy may be appropriate.

i have 1.7 cm complex cyst on one ovary and 1.9 cm paraovarian cyst other side found us last week. no period in 3 years and age 42. all labs and hormone levels normal. should i request for more testing other than follow up us in 6 weeks?

: Follow up ultrasound is adequate. A negative or positive marker for ovarian cancer does not make any difference in the diagnosis.

: Any ovarian cst with complex interior structure should be biopsied.  See your GYN physician who ordered the ultrasound and inquire about laparoscopic biopsy of the right ovary.

Unsure answer: This is difficult to explain without the history as to why the first ultrasound was done and if normal why the second one was done then as well.  Also important whether period is active and still ovulating.  This second ultrasound shows a probable fluid in a cyst with separation but this appears in 3 months so may mean nothing but make sure follow as they are doing.  Need to consult with your Dr.

Complex cyst: Some complex cysts require surgical removal. Some are simple cysts that are collapsing giving them to complex appearance. These can be monitored to confirm resolution without treatment. Ask you doctor to explain the sonogram findings and discuss your treatment options.

Talk to your doctor: The size of your right ovary is about twice the size of a normal ovary and likely most of that is taken up by the cyst. The presence of a thick septation suggests the possibility of more than just a simple cyst but does not confirm anything. Your labs don't really confirm anything and you may need more imaging. Your doc may want a Ca125 test. You need to have a frank conversation with him/her.

Not immediatly: the avascular septum is not as significant as the complex mass which may represent bloody cyst. Repeat sono in one month to reevaluate possible oopherectomy. Other ovary? age?

Advice: At 22 this is unlikely to be ovarian cancer though doctor likely took a full history and addressed any risk factors you may have.  At that size can be watched with serial ultrasounds.  Or if causing pain/interfering with quality of life, taken out laparoscopically. Most common cyst with this description is endometrioma.

my uss report says: the right ovary has volume of 19.2cc's, there is a small complex cyst right ovary containing internal echoes. what does this mean?

SUGGEST DOPPLER U/S: Altho not enough info, i would recommend having a doppler ultrasound and follow the cyst with another ultrasound after your next period. It could go away on its own, but if it doesn't, it could be a dermoid or endometrioma, both of which are benign. Although i would not be too concerned about cancer at this time, keep an eye on it. I do not think that a ca125 blood test is necessary yet.

Typically unrelated: Typically a complex ovarian cyst would not be related to menstrual cycles. Discuss with your doctor if these need to be surgically removed
