Many possibilities: Hard to say just from your description but some answers would be periodontal disease, bruxism(teeth grinding), overly agressive tooth brushing are some examples.   Either a consult with a general dentist or a periodontist would be a good idea.

Treatments necessary: Decay, infected teeth and gums do not stay just in the mouth.  The infection can go to your sinus, or anywhere via your blood stream.  The infection is predominantly bacterial, but may include virus/fungus.  You need to be properly diagnosed and then the infections need to be treated.  Left alone, it will always get worse.  Therefore, your answer is how it should be treated.  Insurance doesn't car.

Yes, and No: Not really. The swelling or infection surrounding a tooth will sometimes give a red to purple zone that can and usually does bleed easily. Dark spots can appear in different sizes and locations for various reasons. Let us know the size, area effected and the length of time that they have been seen. That will help to solve your mystery.

Possible: But not likely. Corrosion of the material will lead to decay--re infection of the tooth. That renewed infection can lead to the formation of abscess, plaque accumulation, gum boils and pus. Bacteria are the primary cause of malodor, but can also be caused by nasal infection and gut disturbances. See your dentist. Fillings don't and aren't supposed to last forever!
