Preserve the molar: If your molar is the first molar, you should try to keep it since it is the most important tooth in your dentition in term of posterior support, chewing function, and effecting the esthetic and longevity of the front teeth.  Any other molar and financial hardship is a concern, extract them.

Tooth pain: Chronic tooth pain can be due to bruxism (chronic teeth grinding and/or clenching), infection of the tooth or gums, a fractured tooth (very common under large silver fillings).
Bruxism can cause TMJ dysfunction with pain referred to the ear.
A definitive diagnosis cannot be made without a clinical exam, x-rays, and additional information.
Check again with your dentist, and an ENT for ear pain.

Fistula....: I personally advise you first see a dentist to discuss your options based upon a clinical exam and x-rays. You should understand the pros and cons of each option and prognosis of Endodontic treatment followed by restorative treatment. Your General Dentist may then elect to treat you or refer you to an Endodontist (or Oral Surgeon).

Return: Please have your dentist/oral surgeon check and see why you are having toothache.  Was your tooth remove to eliminate the pain you are describing?  Was your tooth infected deeply--if this is the case, the nerve inside the bone still needs to heal and it will take a while.  "Toothache" after tooth removal is not frequent, but your dentist may be able to tell you by knowing your specific condition.

Tooth ache: Lower molars and sometimes premolars can cause you to have ear aches. Dizzyness is not likely but remotely possible. Get this checked by a dentist asap.

Infection: Of course, all dental infections are serious. The infection needs to be controlled via the antibiotics. Not sure what your question is. The abscess might need to be drained. If the situation appears to be worsening, consult with an oral surgeon.

Even if you are able to tolerate the pain, the situation could worsen. Please call your dentist. Many come in for dental emergencies or have backup plans for after- hours care.
