: There are many factors that would affect this-like how the injury occurred, what part of the head was injured, how easily the patient bleeds normally, presence of blood thinning medicines, etc. If someone has a head injury and then they develop new symptoms like increased sleepiness, confusion, increased headache, seizure, etc., they should go to the ER for re-evaluation. i had a head injury back in 2019 and my right ear has had a ringing noise in it ever since. i was hit in the head with a metal pipe and it cracked my skull and i had a brain bleed i can hardly hear out of that ear now sometimes i hear a cricket nois?

: What does your neurosurgeon or neurologist think about these symptoms being a complication of your very serious head injury? Have you consulted an ENT specialist for evaluation, advice and prognosis for improvement? Do you wish to obtain a second opinion from specialists at a university hospital near you? If so bring detailed records with you.

Yes: A small bleed can expand, not by re-bleeding, but by absorbing fluid from the CSF (hygroma) and cause symptoms.  This occurs with Subdural / Epidural hematomas

: Most likely, it would show some actual activity.  Whether it would diagnose subdural bleeding, I can’t say for certain.  I think it is best that Neurologist make that determination.

i had an mri without contrast for a 4 day old head injury due to persisting concussion symptoms. my question and concern is if an mri without a contrast given will still be effective in showing any bleeding within the brain?

Accepted Answer

: If an MRI lacking contrast failed to show obvious bleeding, then you thankfully did not damage intracranial vasculature during your head injury.  Unless you worsen with accelerating deterioration, the need for further imaging does not seem apparent.

Exam normal: If your physical exam is normal then no need to rush to head CT.  That being stated, there are a number of nagging problems that can result from even mild injury and seeing a concussion clinic may be helpful.

Not likely: In a healthy adult, it is not likely to cause a brain bleed. This is not the case in an infant as in the shaken baby syndrome, where vigorous shaking causes a brain bleed.

Accepted Answer

No: While in medicine, anything is possible and of course it is possible, it seems exceedingly unlikely to the point of impossible and certainly improbable.  Do you have any symptoms?  Loss of consciousness?  Headache?  Difficulty thinking or concentrating? Nausea? Vomiting?  If you have none of these sympoms, the answer remains no.  If you do have them within 6 hours, perhaps get checked out.
