Different issues: Fibrosan estimates the amount of fibrosis in the liver. AST/ALT ratio is supposed to tell if the liver damage is due to alcohol though there are many other conditions that produce a similar profile.

Difficult question: Why the ursodiol?  If for blood cholesterol, there may be better options (I don't see CF or PBC on your history).  Sounds like you're getting a work up for the elevated enzymes.  Consider a virtual consult with GI specialist for a thorough review of lab and imaging results.

7/1/23: bilirubin 21, alk 119, gt 103, alt 84, ast 44.26/04/23 :b29, al107, gt63, alt54, ast39. 29/07/23: 22, 209,138,68,26. liver enzymes are abnormal for years. all other tests for liver disease and fibroscan show normal results.what is causing it?

: Not clear.  Could be NAFLD.  If they remain elevated for more than 6  months and your hepatitis status is all negative need a liver biopsy.

Abnormal liver tests: There  are many potential causes of abnormal liver tests. Although it is good that your Fibroscan is negative, that does not mean that you don't have a problem in your liver. If your liver tests remain elevated, your doctor should do tests to rule out hepatitis and other causes of liver disease. Be sure to follow up with your liver specialist. Good luck.

ast (23) is greater than alt (20) which i read means cirrhosis, but fibroscan was normal and contrast ct scan showed nothing? what's going on?

As I read it...: ...your AST and ALT are normal, which accounts for why your fibroscan and CT scan showed nothing.  Your stated concern about the AST/ALT ratio may have validity if/when the AST and ALT values are unequivocally abnormal.

i saw 3 liver specialists & 5 gi. none had a pt like myself whose ast(110) is 5x ratio to alt(21). past 2 yrs, my mri, fibroscan&us all normal. i dont drink or take herbs& all my meds reviewed.had any md ever had a pt with ast 5x ratio to alt? thx?

: 3 Hepatologists and 5 Gastroenterologists? - its remarkable that you got your insurance company to pay for all that.  Lots and lots of possibilities - drugs (OTC, prescribed, “recreational” - very common even after all being “reviewed”), autoimmune, NASH, heart failure.  Sounds like you’re very concerned about absolute numbers and ratios.  More important question is whether its progressing or not

Limited usefulness: If liver enzymes are up, finding the ast considerably higher (perhaps twice as high) than the alt is a hint that this may be a problem drinker. A better way to tell is for the physician to make it clear that his/her concern is genuine, and ask.

Depends: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, your results are mildly abnormal. If you drink alcohol, stop entirely and repeat the test in three months. If you are overweight, lose weight. Discuss the results with the doctor who ordered these tests.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.
