An inguinal hernia enters the space where the blood supply (artery/vein) for the testis is present. The hernia can compress these vessels and this might be responsible for episodic pain. yes it can. Nerves giving sensation to the testicle pass thruthe inguinal region and at times irritate them causing pain/discomfort. Good solution to have hernia fixed
