One year post hernia surgery. i get burning pain in groin down to testicle. had every test done. they cant find out why. i get it when constipated before a bowel movement. what could it be?

The mesh : Sometimes the mesh can impinge and sometimes they transect the genitofemoral nerve. This nerve goes from near the mesh to the testicles. Sadly this is not uncommon. Pain management can now freeze or burn the nerve so that you dont get that sensation.
